MEDICATION MANAGEMENT
In 2024, ModUP began collaborating with The University of Florida College of Pharmacy. The goal of The University of Florida College of Pharmacy medication management service is to help ModUP clients get the maximum benefit from their medications through active medication therapy management, identifying, preventing, and resolving medication related issues. See below for a list of services and hours.
Monday: 10:30 a.m. – 12:30 p.m.
Friday: 12:30 p.m. – 5:30 p.m.

- Update primary care diagnosis profile
- Review current medication list
- Medication therapy management
- Manage drug interactions
- Review for duplicate medications
- Ensure medication therapy aligns with each diagnosis (if needed)
- Offer medication consultation and adherence support
- Teach correct dosing
- Assist with dosing schedule
- Teach patients how to manage side effects
- Help schedule follow up appointments with primary care provider
- Smoking Cessation Service
- Assess client’s smoking status and readiness to quit
- Educate clients on the dangers of tobacco use
- Provide treatment options
- Train clients on proper product use
- Track progress
