Full-scale IQ is a composite score from these domains. The average is 100, with a standard deviation of 15. About 68% of people score between 85 and 115.
Can IQ change over time?
Over hours or days, IQ can fluctuate by 2-10 points due to practice effects, mood, sleep quality, or effort. These are state-dependent fluctuations, not true changes in underlying intelligence.
Over years or decades, IQ can change slowly and modestly. Education adds about 2-5 points over a lifetime. Environmental enrichment and cognitive engagement have small, long-term effects. But these changes are measured in points per decade, not per day.
Critical point: A drug that works for hours cannot produce a permanent change in IQ. Intelligence is the product of long-term neuroplasticity, synaptic strengthening, dendritic arborization, and myelination, processes that take months, years, or decades. No pill has ever been shown to permanently increase IQ in healthy adults. Modafinil is no exception.
The meta-analysis that answers the question

What this means in plain language: Modafinil produces small, statistically significant improvements in attention, working memory, processing speed, and executive function. An average person (50th percentile) might move to the 57th-61st percentile on these measures — noticeable but modest.
However, modafinil produces no improvement in fluid intelligence, crystallized intelligence, or full-scale IQ. The effect sizes are essentially zero, and the p-values are high (0.55, 0.89, 0.78), meaning these results could easily occur by chance. Your IQ does not change.
Performance vs intelligence: the critical distinction
Many users report feeling smarter on modafinil. This is real, but it is performance, not intelligence.

Why IQ does not change: IQ emerges from long-term neuroplasticity: synaptic strengthening, dendritic growth, and myelination. These processes take months to years. Modafinil half-life is 12-15 hours. Its effects are state-dependent and disappear when the drug is eliminated. Population matters: who benefits most?
The effect of modafinil is 2-3 times larger in sleep-deprived individuals than in well-rested healthy adults.

Why this matters: Many online reviews and anecdotes come from sleep-deprived students or shift workers. Their experience of “feeling smarter” on modafinil is real, but it is the restoration of their well-rested baseline, not an enhancement above it.

Why IQ does not change: the biology
To understand why no pill can permanently increase IQ, we need to look at the biological basis of intelligence.
Intelligence emerges from the efficient functioning of large-scale neural networks spanning the prefrontal cortex, parietal lobes, temporal lobes, and their connections to subcortical structures. The efficiency of these networks depends on:
Long-term potentiation (LTP) — the strengthening of synapses through repeated activation. LTP is the cellular basis of learning and memory. It requires repeated, spaced activation over days to weeks. A single dose of any drug does not produce lasting LTP.
Myelination — the insulation of neural pathways, which speeds signal transmission by up to 100 times. Myelination continues into the fourth decade of life and is driven by repeated use of specific neural circuits. It takes years.
Dendritic arborization — the growth of new connections between neurons. This process is driven by sustained cognitive engagement and environmental enrichment over months to years.
Modafinil affects none of these processes. It is a wakefulness-promoting agent that temporarily increases activity in attention and arousal networks. It does not enhance neuroplasticity, synaptogenesis, or myelination.
Clinical bottom line: No drug has ever been shown to permanently increase IQ in healthy adults. Modafinil is no exception.
